Workers’ Compensation Procedures

At Liberty Hill ISD, the well-being of our staff is a top priority. It’s important to promptly report any on-the-job injuries to your campus nurse and/or direct supervisor.

[If the incident is life-threatening, please call 911 and notify your immediate Supervisor. Once you are stabilized, please complete the necessary forms from your Workers’ Compensation packet.]

Please follow the steps below to report a workplace injury:

1. Report the injury to the Campus Nurse and/or Supervisor
2. Complete the First Report of Injury & HIPPA forms within 24 hours of injury/incident and return it to your Campus Nurse/Supervisor.
3. Stay in touch with the Campus Nurse/Supervisor and HR Department to provide updates and any forms you receive from your doctor.

• Location List - Assistance for finding a location that accepts Workers’ Comp (if needed).

• Claims Administrative Services Contact Information is Liberty Hill ISD’s Workers’ Compensation Insurance carrier. If you seek medical attention, a CAS Workers’ Compensation Adjuster will contact you directly.

Workers’ Compensation is not job protection. To secure job protection, employees must apply
for FMLA (Family and Medical Leave Act) through benefits@libertyhill.txed.net.

Workers’ Compensation can help cover medical expenses and may provide temporary income
benefits. Additionally, employees have the option to utilize their available paid leave for medical
appointments or for recuperation from a work-related injury.
