• COVID 19 Test Consent Form

Starting 2/23/2022, Health Services will conduct 2 Levels of COVID-19 testing during LHISD Operational Hours.

Level 1: Rapid COVID-19 testing for all symptomatic staff.

Level 2: Rapid COVID-19 testing for Close Contact Staff that have not tested positive for COVID-19 in the last 90 days per TEA guidance.

Consent is required prior to testing.  Please fill out the Google form located at the top of this page.
